Does anyone know of a good guide for documentation. I feel like my documentation is lacking. I usually start at the head and go to the feet. It can be rather lengthy. How much is too much and how much is not enough? Thanks in advance.

Try the following;
The Complete Guide to Documentation ,LWW
Charting Made Incredibly Easy ,Springhouse
Chart Smart, The A-Z Guide for Better Documentation, Springhouse
The third listed is my favorite because it covers areas that don't happen everyday and it is wonderful to have.
