Discharge/Teaching

Hello,

We are currently transitioning to electronic medical record and had questions about discharge. Currently, my unit (NICU) only has a form named discharge summary which basically sums up all the procedures, treatment, and history of the hospitalization stay but has none to minimal discharge teaching. I was wondering what the process for discharge is in other hospitals? Also, if other hospitals have parents sign the teaching log?

    We use EPIC and all the diagnosis for the baby are listed on the discharge page and is printed with the instructions. Our docs still keep a "green"sheet with everything on it and our office transcribes it to a summary and sends it to the PCP.

    We have discharge classes and still do some teaching at the bedside. It is all documented in there. The only things our parents sign are acknowledgment of the visitAtion at the beginning of the stay and the release sheets at the end.
