Discharge date

Hi all, I know I know this, forgive me.. When you have a patient who is discharged by phone today and the last day I visited him was 1 week ago--what do I put down as discharge date. I didn't assess him today, but I am pretty sure the dc should be from my last assessment. TY in advance.

    When you saw him last week what was your plan for him? I'd put the d/c today as today if I were doing it.

    Date of last visit is last week, date of discharge is the date u do the oasis, visit findings are based on the last vist

    Date of last visit is the last time you saw patient. Date of discharge is the day you found out about it. Example: I saw patient on 11/5 for wound care. I called patient on 11/12 and pt stated he does not want to continue services.. My discharge date is 11/12.

    Thank you, all! I called my OASIS mentor and she said "do not put VS in" as I did not assess him the day I dc'd him. In my narrative and summary I put in the date of last visit with my assessment and included in my summary. You all got it right!!! I love this website for us- I always get great advice and help! TY-My mentors online!!
