We do have problem in our count sheet

Unfortunately I couldn't attach our count sheet to see if there is any suggestion to fix it

I would have to send you quite a number. We use the sheets in the sets that list our instruments. This acts as a tool for CS to correctly put sets together and we use them as count sheets for the procedures. Also look at the search for similar threads on this subject.

I've worked as a traveling nurse in 6 ORs and in 3 as staff. At all of the facilities we used the instrument processing sheets that came with the sets also. If there was something missing or extra in the set we adjust the count on the sheet prior to bringing the pt into the room and then inform the processing department of the missing or added item.

what about sponges raytex,marking pen etc..

No, we don't keep count sheets in the pt chart. If the count is off from the initial count I might keep them to send to sterile processing so they know what was missing to start with. That way they can be sure the instrument sets are correct. Sponges and other disposable supplies are counted on a dry erase board in the OR and erased after the case has finished and the patient has been transfered to the PACU. I've never counted a marking pen on my counts, but on big belly cases it might not be a bad idea to count it. Some places have a board on the wall and another small board that is not attached to the wall for things like needles and cottonoids, which can be high numbers. The small board can be carried right up to the field to make it easier to see what you have documented on the board while being right up close to the field.

we have a separate count sheet. Oh how I wish I had a copy for you.

It's a pain in the butt... We've got papers coming out of our ears...tray count sheets, count sheets for the count sheets. it's disgusting & a waste of time.

How I wish we could go back to the tray sets lists, but too many people in too many areas screwing that up or flat out not counting...

ughgh... why can't people just do what they're supposed to? If they would, we probably wouldn't have all this paperwork. It's beyond ridiculous now.
