Consent issue

I have needle phobia with a 5 minute tolerance. So I can tolerate a blood draw. It is only in the arm, 99% of the time my hand is fine. It is a phobia, it makes no sense and I will not "get use to it" double bonus, it triggers conversion disorder in the form of non-epileptic seizures and catatonia. I will require Ativan for a week and aterax for over a month.

what do I need to stop IV in the arm. I have a medical alert, I tell everyone not to, I have a letter stating not to and a rather graphic explanation what happens. I discussed it with the nurse, I discussed all the accommodations that will work, including that if you need an arm IV, give me Ativan first. 
I have been told there was nothing illegal about her placing the IV in my arm. She was even nice enough to document I refused an IV, she placed it anyhow, was forced to remove it, and then documented it triggered a seizure.

what am I missing, because my firefighter training said I'd be in legal trouble if I did that.

I need a way to stop IV placement until there is a need and with accommodations. It's not like I say set a pik line or give me morphine. I have a prescription for Ativan and it is for conversion disorder, so it's not an inappropriate request.
The abuse I've suffered by claiming I have conversion disorder is a whole other nightmare, I have a very, very good reason to do everything in my power to avoid "testing" for catatonia. It is not a claim, I have a positive testing diagnosis.
