CHF Care Plan--Help

Published

HI All,

I am a first semester nursing student and I need help writing a care plan for a pt. with CHF. I just need help getting started by picking the best NANDA for my pt. Problem is the day I took care of him he was discharged. I STILL have to write a care plan on him. I have to have 1 biophysical and 1 psychosocial NANDA and do a care plan on both.

Any help would be appreciated.

Thanks!

Do you have a care plan book? That's the best investment you'll ever make as a student.

you "get started" by sitting down and going step by step through the nursing process. that means working on and analyzing the assessment data, not picking nandas whatever those are, and then determining nursing diagnoses, goals and nursing interventions based upon the abnormal data you discover during your analysis of the assessment data. then, if you still need help, we'll talk.

  1. assessment (collect data from medical record, do a physical assessment of the patient, assess adl's, look up information about your patient's medical diseases/conditions to learn about the signs and symptoms and pathophysiology)
  2. determination of the patient's problem(s)/nursing diagnosis (make a list of the abnormal assessment data, match your abnormal assessment data to likely nursing diagnoses, decide on the nursing diagnoses to use)
  3. planning (write measurable goals/outcomes and nursing interventions)
  4. implementation (initiate the care plan)
  5. evaluation (determine if goals/outcomes have been met)

there is information on how to write a care plan on this thread. please take the time to read it and save yourself a lot of wasted time, wheel spinning, ass-backing and headaches:
