Charge Nurse's, Dealing with Med Errors

Hi everyone,

I am a first year Nursing Student and I am doing research on Medication Errors. I have a few questions and I am hoping someone here who is a charge nurse or supervisor will be able to answer them. They are strictly for research, completely anonymous. Nobody will know who you are or who you work for. If anyone can help me these are the questions:

1. What is the frequency of medication errors in your area of specialty.

2. What are the nursing responsibilities when a medication error occurs?

3. What is the specific role of the nurse supervisor (e.g. writing counseling & corrective measures) when a medication error occurs?

4. What hospital committee makes decisions concerning medication errors?

5. What advice do you have for a new graduate?

6. How are medication errors traced by nursing to find causes and improve client care?

7. What do you feel is the most important aspect of being med savvy to learn in Nursing School to bring to the real world later?

8. Is there any literature or resource regarding Nursing in general that you feel would be of interest to someone just starting his/her nursing career?
