You need to begin the process entirely from scratch, not sure what has changed on the application since then, so I suggest that you download the newest set from their website. You could not do that four years ago..........

No i mean that i will do the same process from the begining? i have a new report from the ICD center because i am still studying at the university of Phoenix, what do you think?

Yes, the entire process needs to be repeated. The application and all of the documents are only good for two years. So, if it was done four years ago, they no longer have the file available.Again, use the newest forms that are out there. There have been a few changes that may effect you.
