Case Management

Our agency went to a primary nurse case management model at the beginning of the year, shortly before I started. I came from an agency that also used that model, so I am an old hat as both a field clinician and a manager. I have a good understanding of the model and feel very comfortable with it. However, most of our current staff is struggling with it and I am at a loss for how to handle this. I have tried educating at case conference meetings, but it seems to be falling on deaf ears. Our "educator" has been asked repeatedly to address this issue, but nothing has been done about it as of yet. I would like to put together a brief inservice for nursing and therapy regarding primary nurse case management and was wondering if anyone here has a written description of the responsibilities of the RN Case Manager and how that role is fulfilled by an individual. I would love pointers to journal articles if anyone has any and will happily provide my email and/or fax number via PM for any documentation you care to share.
