Care Plans

OK, I looked this on up in my care plan book and here are the Expected Outcomes Listed:

Patient will regain skin integrity--for ex. pressure ulcer will decrease by (a specific size)

Patients wound will heal

Prevent skin breakdown

Patient will communicate understanding of skincare measures.

Patient will perform skin care technique.

The thing about these dreaded care plans is that they are individualized--so you have to take the expected outcomes and tailor to make a short term goal. Our instructor wanted us to make it pertain to the time we were there. So is that is your case, the wound will remain clean, dry and free from infection. No further signs of skin breakdown as evidenced by increased erythema, etc.

Having said that--all instructors want them a little different so maybe you can find a senior student in your program and ask them.

Hang in there!! The care plans do get easier and SAVE them all, because once you turn in one your instructor likes, you can just tailor it slightly for the next patient if they have the same dx--knowing it has passed the critique already.

For a short-term goal I would go with something like will remain free from signs and symptoms of infection over next so many days, then specify what s&s you would look for. That or will state pain level of 3 or below over next so many hours. Those were the first that popped into my head. Good luck getting the care plans down though, they take some time to get used to.

invest in a good care plan book i have one by ackley and ladwig it is awesome it has got me through until now basically someone told me your short term goal is going to be something you do to help w/ the problem while you are on the floor during your clinical like pt. will practice rom exercises at say 4 and 6pm or pt. will express ways to keep skin intact at 4pm make sure the goals are measurable so put a time frame. for your long term goal something like client's skin will remain intact by discharge date aeb: no erythema signs of breakdown etc...

i still have trouble w/them but that book is a lifesaver i swear
