HELP! I am a new nursing student, started last week and we have a care plan to write, i know some day this will be a lot easier (also a lot faster) but right now its taking me quite a long time.

pt is a 24y/o female, 2 children (3yr, 6wks) reporting hearing loss in left ear. She also is having feelings of fatigue, depression, and clumsiness. Last piece of info she gave me was that she had a diagnosis of MS 8 years ago but did not follow-up with MRI to confirm.

What would you address first? her hearing loss or getting a definitive diagnosis of MS???

think if you were in her shoes.....what would you need first....what would be most important for you.

I would want to have my diagnosis confirmed first for MS, but that is me. Clearly this patient is having difficulty with the diagnosis since she never had it confirmed in the 8 years that shes known. But my gut tells me that since she came in for hearing loss that that is what I should address first...

28 yo female witha toddler and new born with c/o hearing loss, depression, fatigue and gait disturbances (clumbsiness)

Look at the pryamid and your senario.......

is she eating, have shelter, the ABC's (airway, breathing, circulation), being hurt have help at home

next,

security of body,the family, health, resources......she's 28 with a toddler and a newborn experiencing physical problems

Is she being hurt, she mentions depression...is she suicidal, are the children safe, is she safe. Does she have help? Is she being abused? Is she falling? has she dropped the baby? Is this post partum depression or MS?

Clumbsy makes me think of safety for her and the children. Is she falling, dropping things...She mentions MS and didn't follow up....why? Is she in denial?.....Does her family help? DO they know she needs help.....

I would think her clumsiness/hearing would come first as she is taking care of children...the clumsiness can be a tell tale sign along with problems associating in the ear (I.e. Vertigo)... You want to address the interventions you can perform on her. You won't be allowed to diagnose her with M.S, but you can make your own nursing diagnosis about everything else.

Her depression (possible post partum) should also be addressed... But the physical ailments should be concerning first for her physical health.
