Care plan practice?

Does anyone know of any websites or books out there that offer sample patient charts that would allow for one to complete a practice care plan? I know, it sounds masochistic, but I have some time off from the accelerated BSN program I'm in, and the fall will be heavy on clinicals and care plans, so if I can practice some more, that would be great.

Any guidance would be greatly appreciated!

these kind of take you through a step-by-step approach. they were done by the nurse educator of a hospital.

i would suggest that you practice with some of the nursing diagnoses that are used most. for example decreased cardiac output is one that gets used a lot for congestive heart failure and cardiac problems involving arrhythmias. the pathophysiology of chf and the way the electrical system of the heart works needs to be understood to use this diagnosis correctly. pulmonary problems are also big, so ineffective airway clearance, ineffective breathing pattern and impaired gas exchange get used a lot. the nanda taxonomy is printed in the appendix of the hard cover medical dictionaries and it is important to know this information. you can also access some diagnoses on these two websites:

Thank you Daytonite!!! I was sitting on my front porch the other day thinking about school starting in 2 weeks when I realized I had forgotten HOW to do a care plan. Seriously, I had to do 3 of them a week last semester-how could I forget? I wasn't completely "proficient" with them until the last month of clinicals when it all "clicked" but I ended up loving to do them. I am going on the links you posted today so I can get some practice in before school starts. Thanks!!!
