Care plan HELP

I am making a care plan for someone who is in the later stages of dementia and every morning they cry while doing morning care. So I am doing it based of of fear related to deminta. My question is how do I make a nada statement.

"Coping response of fear related to dementia. Evidence by crying during morning care, etc" Would this work, any help would be much appreciated

So you have to pick the NANDA approved nursing diagnosis -- Ineffective Coping

Then you have to pick the medical diagnosis that it's related to, in your case you picked dementia:

"Ineffective coping related to dementia"

Now you have to support why you related it, with an "as evidenced by" statement:

"Ineffective coping related to dementia as evidenced by crying during morning care"

Fear and anxiety are also NANDA nursing diagnoses you could use, if those are the primary sx you have assessed.
