Care plan help!

I need help with my nursing care plan. I am also filling out a physical assessment data collection form and would just like maybe a sample to see if i am doing it right. Know any good sites? careplans.com costs money and i dont want to pay for just browsing. Thanks!!!!:bow:

you will find a sample write up of a doctor's history and physical exam on http://meded.ucsd.edu/clinicalmed/write.htm. there is a nursing physical assessment form in the center of taber's cyclopedic medical dictionary under the heading of nursing.

i posted a list of examples of care plans that people can view on this thread the other day: https://allnurses.com/forums/f328/care-plans-349248.html. if you think, however, that care planning involves copying them from a website or a care plan book, guess again. that is only going to work for you if you luck out and get patients who have the medical diseases that the care plan books happen to address. the fact is that people have many other medical diseases that don't show up as care plans in the care plan books. then what? hope the care plan fairy comes around?

there is a method and process to the writing and construction of care plans. it involves the incorporation of the nursing process and critical thinking. i focus on it in the writing of care plans all the time. nursing care planning made incredibly easy is based on it. even nursing diagnosis handbook: a guide to planning care by betty j. ackley and gail b. ladwig spends the first section of their book on this idea. it is because if you get a patient who has a disease such as polio you will be able to put a care plan together for them because it is very doubtful that you will find a care plan for polio in any care plan book.
