care models

i was asked what "model of care i support " by my precepting student for a paper she is writing. i felt a bit old fashioned responding that

individualized care models have always been the way i deal with inpatient and out patient mental health clients. for a few days after i answered that question i have pondered if what the words "individualized care" now means what it did when i started nursing in the mid 1990's. canned tx. plans, very similar nursing notes, doctors checking a 3x5 cards to make sure the right dx. hits the right medicare fee schedule dx ... remind me again what care models we are using these days. and are there any that utilize preventative measures, patient teaching at every turn and competent professional care that includes just plain listening (even for a few minutes) to our patient's concerns.

more importantly, what was the right answer?
