CA nclex test

hi there! I had my CA nclex last april 2008 and failed. now i want to take it again. do u think my records are still in the ca bon and i will just need to send reapplication form since it's already 2 yrs since i had my last exam.and if i will send reapplication form should i still use the address i used when i send them my form or can i use my new address. pls help! thanks!

    Contact them to find out if you need to send a new application, either way use your current address.

    ok so I called BON. they asked me for my SSN and I said that I still don't have it coz I am still on my adjustment of status here in states. so I should call them the next time I can provide them with my SSN and they will tell me if I should just send reapplication or should I send them new application form. I also asked if ever i need to send new application form, can I use my new last name since i got married and said yes provided I will send a copy of my marriage certificate. there. so anyone have this problem then here is the answer. :)
