Brain Sheet for Clinical

Does anyone have a "brain sheet" that they use for clinicals to ensure they have everything? I would loooove one! ?

    This is one I doctored up based on one I found online (I think from here, LOL) It's good for med/surg. Originally when just getting 1 pt, I'd print it full size on one page (landscape orientation) But now with multiple pts, I'll print it 2 to a page, portrait orientation. You've gotta write small in that case!
