I have to do a presentation on effective communication and was hoping to find some good examples of bad charting I have heard some over time but now need to recall some of it for a paper anyone know where I might find some decent examples.

I think Kayciel is a brand name. Our MD's usually write for K-Dur (extended KCL PO), K-chlor (reg PO) or KCL IV.
