You are applying for licensure and there are a couple of threads on both Vermont and New Mexico and there are links with both of them on where to apply for licensure.

There are no applications that exist for the NCLEX exam. You must go thru the License by Exmaination process for which ever state that you want to get licensed in. You need their approval to be able to sit for the exam.

Suggest that you make a decision as to where you want to work first, and not who is the fastest. If you are not going to work in one of those states, then it can be longer to endorse, and also much more expensive.
