Welcome to AN! The largest online nursing community.
We are happy to help with homework but we need to know what you have found out first. What do you think are the elements of risk assessment. what risk are you assessing? Medication? Behavioral? Suicide? Violence? Drugs/alcohol? Safety? Detox? Confusion? Falls? Heart attack/stroke? Abuse?
What risk are you assessing?

if i say i am at risk for creeping obesity, say, what would you want to look at that might increase my risk? proximity of tempting high-calorie foods? lack of exercise (because i'm always on an, or have an amputation, or a psych problem that makes me afraid to go out, or sheer laziness and addiction to soap operas)? lack of knowledge about proper nutrition and exercise, because i am illiterate? how would you assess those things which are risks to make me fatter?
that's how it works. you look at your person or population at risk (me), what they're at risk for (getting fatter), identify applicable risk factors (sloth, gluttony, ignorance, illiteracy, fear), and then work out a plan to mitigate (decrease) those risk factors, which then decreases the risk.
this is sort of a tongue-in-cheek example with exaggeration for effect (don't even think of submitting it as homework, it's a d- fer sher), but that is how the process is done. what have you come up with?
