Admission forms question

I am having some issues with the skilled facility my mother is at. In looking over her medical records I think there is some missing. Could someone tell me exactly what forms need to be in a chart when first or shortly after being admitted?

If you are concerned about your mothers care you really need to speak the the director / person in charge they will be able to investigate your concerns appropriately

This is not something that we are going to be able to answer in the forum as the previous poster has indicated it will depend on the facility.
