Hi everyone! I recently started a job as an RN on an ACT team. I found a thread on this but it was pretty old, so I thought I would make a new one. I was just wondering who else was on an ACT team? If you are, can you tell me a little bit about your team? Thanks!
I'll begin. We have a little under one hundred patients and our most common diagnosis is schizoaffective disorder. Some of our patients are very high functioning and have families and jobs but a lot of them require more intensive services and need transport and money management.
We have one psychiatrist, two RNs, two case managers that are master's level and three that are bachelor's level as well as our team leader who is an MSW. One of our bachelor's level case managers is a housing specialist, another is an employment specialist and the third is a drug and alcohol counselor.
As for the two RNs, our duties consist of medication management and coordinating care (making doctors' appointments and such), obtaining monthly vital signs and annual assessments, administering long-acting injections as well as a lot of case management. We are sometimes in the office but other times we are out in the field.
