I would try to make sure that my practice everyday was above reproach the best I could. Then.....Depending in the situation, I would document, document, document......then I would contact my malpractice insurance and contact a lawyer.

It's just a hypothetical question. I would talk to my charge nurse and explain what was going on. Then proceed with the best care possible. I would also have another person go into the patients room with me whenever I need to to enter and document as you have mentioned.
