I'd like to take the CNA test in Arizona

I am a nuring assistant in Phoenix AZ who has taken a local community college course for CNA and have been working as an unlicensed NA for almost two years now at a local hospital. I am also studying to be an RN and I am currently in Block 2 of nursing school. I haven't taken the CNA exam yet - been very busy with work and school but I'd like to get my CNA cert in case I want to move within the hospital where I now work. What do I need to do in order to take the exam? I have the letter from October 2009 certifying that I have successfully completed the 120 hour CNA course at Glendale CC and I have been working as an NA since Dec. 2009. Should I check out the AZ State Board of Nursing website for this?
