Hello. Does anyone know if I can download a copy of the "Certification and Authorization Form for Criminal History Background Check" off the internet? The NJ Board of Nursing has it in the nursing license application on their site, however it appears that there is a page missing from the document as it says "continue on reverse side", however there is no next page.

Well I requested the application packet in the mail since I was in FL at the time and same thing with the hard copy they sent, it said continued on reverse side, but there was nothing. So I don't think there is one!

Maybe someone else knows for sure...

I have done the entire paperwork thing, took my boards and passed and I wanted to let you know that there is a second page, it just says "Certification" at the top and you have to sign it. That part does not have to be notarized.
