What are some limitations with having a derm NP solo practice in Cali?

My parents own a beauty clinic currently and they want to give me the practice as they are getting older. I figure it would be a great idea in the possible future to become a NP specialized in derm to increase the scope of practice of the clinic. I would probably get my esthetician license as well. I would assume it would be a mostly cash based clinic and probably not difficult cases. Now I live in the state of California which is not autonomous for NP.

So what limitations/hurdles could I expect when running such a practice? Legal issues, reimbursements, etc... I assume I would need a supervising MD to review charts and such. If there are insurances would there be any issues with reimbursements due to it being mostly run by a NP? I have also read that the nursing lobby has tried to pass a bill to allow NP to practice autonomously in California, but failed. From your prediction, do you think this will change in the near future?
