Where Can I get a good Nursing Care plan book?

I am looking for a care plan book that gives you the interventions with the rationale. If anyone has 1 they really like I'd appreciate it. Thanks!!:clown:

I have Cox's Clinical Applications of Nsg Dx and I am not crazy about it. I did use Sparks & Taylor which I did like better but unfortunately I only had limited use of it so I can't say it was much better. But take a close look at it and see which suits your needs.

Cox's was very good with rationales, though.

I bought Nursing Diagnosis Handbook; An Evidence-Based Guide to Planning Care 8th edition by Ackley and Ladwig last semester. I helped a lot. It was actually a requirement for us; came on our PDA software, but the book itself is more complete than the PDA software. In the front of the book, you can look up different illnesses or issues, and it will have a list of nursing diagnosis to choose from. Then the rest of the book lists each of the individual diagnosis along with the interventions, etc.
