Visascreen Application

Hi, I am an international applicant from the Philippines, seeking guidance for my Visascreen Certificate Application from CGFNS.

I already passed my TOEFL Exam and NCLEX exam under Nevada BON. I have already paid my expenses for the Visascreen application fees ($540) as well. As I was about to print the required documents to be accomplished in CGFNS after paying, I have observed that I lack one of the documents needed to Validate my License/Registration in Nevada Board of Nursing. I only had the customized form of "Request for License/Registration/Diploma" which is addressed to my local board of nursing authority (Professional Regulatory Commission). I know that this form is also needed to be accomplished in order to validate my license here in the Philippines. What I was looking for my is another form of "Request for License/Registration/Diploma" which is addressed to Nevada Board of Nursing in order to validate my NCLEX Pass Results there. What should I do? I really do not have this important document to pass to Nevada BON. As much as I have remembered while I was encoding my details in the Visascreen application, I was able to declare in some questions that I have passed an NCLEX Exam, specifically under the state of Nevada Board of Nursing. That's why I do not know why I lack this document in the "Print Documents" section which should be there automatically.'In dire need of your guidance and help. Would appreciate it a lot.
