Which answer is the best? I chose C, but the correct one is B. Why?1. Just think about the question. While C may be correct to some degree, the question said BEST answer. When looking at the choices that are available it's very obvious that B is the best answer. When you ask someone for their history, why would you be asking?? Think about the nursing process (think assessment). Are you asking only to let their doctor know? NO! You need the history of the patient so you can meet their needs--develop a plan of care and this helps to guide your interventions, determine who you should consult, etc, etc.
