Teaching Case Managers Role In Home Health

Does anyone know of where I can obtain information on this topic. Have new nurses and some who have been doing homehealth for less than a year and they are struggling with the case managers concept. They are great task nurse but need to help them understand the entire role. Thanks

I recommend the book by Tina Marelli: The Handbook of Home Health Standards: Quality, Documentation and Reimbursement. It has a lot of information about the role of case manager.

I have been putting an "article of the week" in their boxes. Subjects have been fall risk assessment, mini-nutritional assessment, signs that your patient could benefit from therapy, documentation with examples, medication review and polypharmacy in the elderly, environmental assessments, and once even the definition of case management from the CMSA. They seem to be coming around!
