Section M question

Hi !

I am pretty new to MDS and have a few questions..I apologize if this has been asked before. First question, Do you code vitamins under section M under Nutrition/hydration?

Second, Is excoriation considered a stage I? This has been going back and forth between the staff.

Do you code vitamins under section M under Nutrition/hydration?

If the management of a skin problem is attributed to a nutritional deficiency due to a lack of a particular Vitamin intake (preferrably substantiated by a clinical dietitian), then the Vitamin supplement can be coded in Section M1200D. Any other vitamin supplement is not.

Second, Is excoriation considered a stage I? This has been going back and forth between the staff.

Stage I is only a reddened but intact skin. An excoriation is a worn off skin from irritation or friction. If it were on a pressure site, Stage II is more like it. However...

Coding Tips

• A Stage 2 pressure ulcer presents as a shiny or dry shallow ulcer without slough or bruising.

• If the oldest Stage 2 pressure ulcer was present on admission/entry or reentry and the date it was first noted is unknown, enter a dash in every block.

Do not code skin tears, tape burns, moisture associated skin damage, or excoriation here.

• When a pressure ulcer presents as an intact blister, examine the adjacent and surrounding area for signs of deep tissue injury. When a deep tissue injury is determined, do not code as a Stage 2.

It can only be entered as M1200Z.
