schizophrenia

does anyone know where i might find a journal article on schizophrenia or the managment or treatment of teh disease???? i also need some thoughts on my nursing dx and interventions... my pt is only in the mental health facility b/c his doctor changed his meds and the dose of the new one was to low and they're just trying to get his med reginmen back to normal.. and it is b/c he's being d/c'd tomorrow.. but i still have to have a dx and stuff so here goes...

Interventions... this is where i'm really stuck b/c theres not really much to do for him! i thought about putting "Educate about the need to take his medication ever day and at the correct time." ... i know that probably sounds dumb...any insight would be greatly appreciated!!

if the pt is being discharged it would be really important to assess his insight and judgement, or lack thereof (that's another pt problem on its own). Knowing this will tell you whether the pt will understand comply with meds and will tell you if he/she trully is ready for discharge.

how did you choose this nursing diagnosis? what defining characteristics (symptoms) did the patient have that qualified him to be classified with this diagnosis (what's your aeb information?)? [every nursing diagnosis has a set of defining characteristics (symptoms). you cannot assign a nursing diagnosis unless the patient has at least one or more of those symptoms.] here is a webpage that includes the nanda information for this diagnosis: [color=#3366ff]disturbed thought processes.

if you understand the nursing process, then you understand that your patient's defining characteristics (symptoms) that prove the existence of this problem are what you develop your interventions for. in other words, you treat the symptoms the patient has. your treatments will be your nursing interventions.
