Help...i have a question for my class that i am stumped on. A 22 yr old who has a hx of seizures and thrombosis comes into the clinic for oral contraceptives. write an RX for your answer...i thought that you weren't supposed to give BCP to those who had a hx of thrombosis? am I wrong...??or is there a safe alternative???I know you want one with low estrogen..help...also someone who was just diagnosed with parkinson's disease(67 yo). ??thanks in advance,
First off, this isn't my area of expertise, so I am just throwing out an idea here. Perhaps this is a case where it is your job to educate a patient as to why the contraceptive she prefers isn't the best option for her.
Secondly, why would a 67 year old patient require contraception?
If estrogen is the problem both with thrombosis and with most seizure controlling medications, her only option would be a POP - progesterone only pill, which has its own limitations, and must include counseling about same time daily dosing, etc. And the previous poster was right - it might be a "counseling" type question - why do you particularly want to use the pill, why not DMPA, an IUD, condoms....
And I know you can find the answer about Parkinson's - its diagnosis is confirmed with a trial of this medication.....
