Retaking NCLEX

Hi everyone! 
1. I graduated in 2020 I failed the NCLEX in 2021 and retook it in 2021. I have not retaken it since 2021 am I still OK to take it?

2. I sent in the application to retake (I'm from pa and we still can't do it online)with $75 over a month ago I also had to change my last name and address so I sent everything together. I received an email about the last name and address being changed successfully. But have not heard anything about the application. On my PALS nothing even says pending. It's been over a month I have tried to call and sit on hold for over 3 hours. I've emailed and get no response has anyone else had this issue?
