Quality Improvement Issues

What do you think are the biggest nurse-sensitive quality improvement issues in Surgical ICUs? I am a senior nursing student and I'm currently doing my practicum in a SICU. I am going to write a research paper, and I know the obvious choices: VAP, DVT prophylaxis, etc. I'd really like to research something that not everyone and their brother has already researched.

Any ideas? I'll let you know what I come up with re: best nursing practice :)

This is probably too late for you but other things my SICU looks at besides those things are --

1. Extubation times after CABG surgery (faster is better)

2. Pressure ulcer incidence/prevalence

3. central line infection rates

4. phlebitis rates

5. UTI rates related to indwelling catheters/foleys

I work in an SICU and our biggest problem is VAP....

OUr off pump cabg population is coming out extubated... alternatives in pain control as to not depress resperatory status is an issue.
