  1. He came in approx 15 days after abt was administered.. still complaining of burning while urinating.. When I say follow up, he did not show up for his initial follow up... but made a new appointment for the issue not getting resolved.. sent it off & today he got his second round of abt. I was not the initial triage nurse.. just treated when he came in the second time & today.. still pretty puzzled, since he seemed really insistent he hasn't been with anyone & took all the first round
  2. When asked about sexual Hx it was a one time thing with alcohol involved, when asked by Doc if his partner needed Tx he had said he didn't even know how to contact her.. to my knowledge & his "honesty" it was a casual encounter, I think hence he was doc adamant about not sleeping with anyone since his first round of tx.
  3. I had a pt who came in, tested positive for Chlamydia.. he got the usual Azithro 1g for Tx, he came back in for his f/u and his urine still tested positive for chlamydia.. Doc prescribed another round of ABT, however Pt swears up and down he took all the pills and that he was not with anyone, he did tell me he was taking anabolic steroids... for my own knowledge and curiosity.. does anyone know if steroids affect the treatment of illness? I'd imagine so, but I can't think of why.. I'm a newer nurse sorry if this is a stupid question it's just bugging me to know the answer
  4. I'm a new nurse in California and not sure what to type into google for the answers im looking for... the run down: I am newly working single mother of a 3 year old in urgent care and the job I had applied for is located approximately 25 minutes from my house, which is no problem for me swinging childcare arrangements for an 8 hour day. My boss has called me the night before asking me to go to another urgent care location of ours that is an hour and a half without traffic away from my house. anywho, I am ranting at this point haha, I am curious of commute pay in California and required overtime for nurses.. I've worked a little over 80 hours for 9 days straight and have commuted twice to further away locations that I did not apply for, boss was just NOT taking no for an answer, offered an uber, childcare (which I'm not comfortable with i have my family watch my daughter for a reason) and a hotel room for me & her (not comfortable with) because I did not want to wake my daughter up really early.. in the end she had promised a bonus so I had made the early arrangements and commuted over an hour and 30 minutes than I usually do there and back all to see on my paycheck there is no drive time, and no overtime pay. Im curious as to what the overall rule for nurses are for drive time and overtime is before I bring it up & how professional/legal is it to NOT have a set schedule, because I can't keep always being on call until she calls me the night before at 9:30-10:00 with the surprise location for the next day.. if it was was just me the scheduling wouldn't be that big an issue but I have a small child and my family is on call just as much as I am just in case i need to arrange/change something last minute Sorry it's so long just incredibly frustrated Rachel
  5. A coworker who had been there a lot longer said the "loophole" and what made it within our scope was that the patient is "stating" what it is to us, and we type it into the iPad, thus making is Subjective data in our SOAP notes.. very confusing, and hoping to get better insight from an outside source as I am a newer nurse and have only worked in LTC until this job
  6. Also to be clear, when I say establish a POC it is with the physician as well, not a decision of my own. I just carry out orders like shots, wound Tx, blood draws, orthopedic wrapping and slings, and other things.. everything I can carry out is in my scope, it is the registration, admissions, assessment, insurance & billing referrals and follow up I am concerned about lol
  7. Thank you so much for your reply, this is my first week at this company and I have already worked 9 days straight and a few of my new hire co workers are concerned as well. We've looked up our scope and talked amongst ourselves about it but aren't but entirely sure how to take it up with our bosses. On top of it, a couple of the clinics are understocked as in has NO sharps, no rapid tests and they have told us we do not need the rapid tests to Dx a patient with strep or flu, and have had doctors prescribed ZPAK without an official Diagnosis of strep. I didn't prescribe it so I'm not worried about it but I did lent it be known I had no rapid tests in my clinic and just described what I saw based on this "initial assessment" so...
  8. I've recently started working at a TeleMedicine Urgent Care clinic here in CALIFORNIA.. I am an LVN and I'm concerned the work my boss is having me so is not under my scope of practice.. the rundown of this place: I am the only nurse on hand, there is no MA, no Physician or second nurse in the clinic. I have an iPad with the app Dr. Chronos on it where I was trained to register new/established patients, document chief complaint, history of present illness, family and social hx, a complete initial physical assessment and I am expect to run standing orders based on their CC.. After all of this admissions stuff I am doing by myself, I log into a website called ChironHealth where i am able to connect with a provider (MD,PA,NP) at another location and create a virtual appointment for my patient, I sit in the room during the appointment and scribe SOAP notes and then establish the plan of care & carry out the orders.. my giant issue is a HUUUUGE chunk of this process I have to do is out of my scope of practice legally according to BVNPT.. has anyone else expirence a scope of practice issues or telemedicine & tell me if there is a loophole? My boss has no medical background & I don't think she understands anything I try to tell her.
