I want to make sure I am understanding this correctly. Am I doing this correctly?? ORDER: VANCOMYCIN 0.5G IV Q12H FOR AN ADULT PATIENT AVAILABLE: 500 MG/VIAL Preparation and stability At the time of use, reconstitute by adding either 10 mL of Sterile Water for Injection to the 500-mg vial sterile vancomycin powder. FURTHER DILUTION IS REQUIRED. After reconstitution with Sterile Water for Injection, 5% Dextrose Injection, or 0.9% Sodium Chloride Injection, the vials may be stored in a refrigerator for 14 days without significant loss of potency. Reconstituted solutions containing 500 mg/10 mL must be further diluted with at least 100 mL of suitable infusion solution. The desired dose, diluted in this manner, should be administered by intermittent intravenous infusion over a period of at least 60 minutes. What is the dose strength of the reconstituted vancomycin? would it be 500mg/110ml because we are reconstituting with 10ml of sterile water and we are required to add an additional 100ml. How many mL will you administer/ dose? 0.5g * 1000mg/1g = 500mg 500mg*110ml/500mg =110ml 110 ml i dont believe this is correct
