  1. I start my transition clinical in about a month. We haven't been assigned to a site yet. I'm sure this will be last minute like everything else in this program. We're supposed to write a letter to our preceptor with all the generic information. We are also supposed to have 4 goals. I'm have my goals written, but I'm unsure if they'll be considered S.M.A.R.T. friendly. I can rock goals in a care plan. Real life? Apparently not haha These are the goals I came up with. I'm also open to ideas you may have if you think I could choose a better goal. Be able to discuss disease management and educate patient and family members correctly by the second week of clinical. Improve my time management skills by appropriately delegating work and becoming comfortable delegating work by the end of clinical. Improve my hand off communication skills to provide an accurate, thorough, and concise report by the end of the first week of clinical Become more comfortable contacting physician's regarding patient information, or changes in condition by the end of clinical Thank you for taking the time to look/help
