  1. I have been brainstorming of all the components that I wish my brain sheets would have for the past year or so and I finally had time to put one together! I am new nurse and this has been helping me in not forgetting to ask or assess anything as well at report time, it helps to keep my thoughts organized. I typically have 4-6 patients that I care for (ALL care - baths, toileting, transfers, assessments, feeds, drsg changes, meds, wound care) so my day can get quite chaotic very fast and I despise the feeling of being disorganized. This helps me organize my thoughts and what I have to do for the day. I have the brain sheet below on a LEGAL sized piece of paper and it fits 4 on each side (I print it on both sides in the event that I have discharges, new admits, pickup patients, or that we are short and I have more than I usually do). I typically highlight anything that I need to do for each patient - e.g. I highlight my blood glucoses (BG) if they are diabetic. I also light to highlight the med times so that I can reassure myself, without having to flip through each MAR, that I do/do not have meds at lunch, etc. I thought I would share it in the event that it could help someone else as much as it helped me.... so ENJOY! xox :)
