Hello, I am filling up the Request for Certification or Verification of New York State Licensure form and there is this item that really confuses me. It is under Item 2: Licensee information, It is asking about the "Name Originally Licensed Under". I am not sure if I'll put my name under which I was licensed or the name of the licensing authority. Thanks! BTW I am applying for a visa screen.

Hello, I am also processing my Visascreen and I would be grateful if you could answer a few questions below regarding my application. By the way I am from the Philippines and I took my exam last September 22, 2015 and after 3 days I found out that I passed through the quick results. Questions: 1.) Do I still need to wait for my NY license to be issued before I could submit and pay for my application? 2.) Should I still send the Request for Academic Records/Transcripts and the Request for Validation of License/Registration/Diploma to my school and licensing board even if the CGFNS already validated my credentials through the Credentials Verification Service for NY? 3.) On the Step 6 (Professional License Information) of the application process, should I include the license that I acquired after passing the NCLEX-RN under the New York State or just my license here in the Philippines? Your help will be much appreciated! Thank you!

Hey guys! I am from the Philippines and wanted to start processing my Visascreen application but the problem is I still don't have my license which I think that is needed for my visascreen. Does anyone know when will I see mine online? I took my exam last September 22, 2015 under NY and after almost 3 agonizing days I was able to avail for quick results and found out that I passed. Thank you! P.S. I already took the NY mandated courses and had the certificates sent online.
