  1. Hi everyone, I'm a medication aide working at an assistant living facility and I need some pretty serious advice. I was made aware of two medication errors that have been happening for multiple days which I reported to my superior. I was expecting a pretty big reaction to these multiple medication errors since it went on for more than two weeks and was completely do with not going by the MARs while passing out medication. I even expected this amount of errors to some how attract attention from the state but when I spoke 4 days later to my superior I was told they were still "trying to figure things out". I requested that the patient, doctor, and state be informed but to my knowledge only the patient was told. I haven't seen any doctor's documentation on the subject and the medication aide that made the multiple errors is still employed and passing out medication. The doctor and state could have been informed but at this point I am worried that they downplayed the errors or didn't report anything. I have a letter from my superior saying that they have dealt with the issue but I am finding this hard to believe. I feel like I'm in over my head. I want to report this to the state but have no idea on how to go about this. Is there any advice some nursing professionals could give me? Any route I could follow? It would be much appreciated.
