What I did with mine is to let somebody fill-up my form who have a very good handwriting. Then I handed over the form to my employer/reference/doctor to just sign their signature. It works.

Is it 50 now? The last time I know it is just 10 critical questions.

Did you guys sent the form Training Area reference form? Why others are sending it. And one more question, is it possible that my NMC portal be re-opened? I overlooked some informations that I gave on the portal. Has anyone tried it?

Guys, i need confirmation...this is regarding the declaration of health...mine is already signed by the doctor of the hospital that i am currently working with and already have a dry seal in it...is it required to still put the license number of the doctor who signed it and where?
