I am having a little trouble with nursing diagnoses. I was just looking for a little guidance and wanted to check if I was on the right track. My Diagnosis is: Impaired gas exchange secondary to anemia as evidenced by shortness of breath with any exertion. HGB is 9mg/dL I need an assess, do, teach. I was thinking my interventions would be respiratory monitoring and oxygen therapy. Assess: I would assess the lungs for areas of decreased ventilation and the presence of adventitious sounds. DO: Position patient with proper body alignment Teach: appropriate breathing and coughing techniques Assess: I would monitor for changes in BP and HR Do: change patient's position every 2 hours Teach: Explain the need to restrict and pace activities to decrease oxygen consumption during an acute episode. Or should I assess Oxygen therapy? use a pulse ox to monitor for oxygen saturation Do: Teach: Home health services for nursing care or oxygen management. I'm completely lost with this. If anyone has any advice or any kind of direction I would appreciate it. Thanks in advance.
