  1. I am making a care plan for a patient I had in clinical with ESRD who is on dialysis. I know that the number one problem after someone goes to dialysis is their risk for hypotension. I cannot come up with a nursing diagnosis that works for that. So far I have: - ineffective tissue perfusion r/t ESRD - Excessive fluid volume r/t decreased urine output - Risk for infection r/t AV fistula site Are these diagnoses good for an ESRD patient? and does anyone have any suggestions for the hypotension risk?
