Going from LTC to rehab with 5 months experience. Is it an easy transition?
I wonder if anyone has work at a LTC OR SNF and changed over to another? Was the transition easy or did it take a while to get to feel comfortable? im looking for fairly new nurses at fill me in. I've been on LTC for 5 months but I'm moving to a SNF some with rehab patients. Part time will have LTC/ rehab and the other times will be just rehab.

MAR how should it read?
MAR (paper chart) reads: Medication name 10mg 1 tablet Medication sticker from pharmacy reads: medication name 5 Mg 2 tablets I understand I'm supposed to pop out two tablets becasue the dosage is not sufficient to equip 10 mg with one tablet but shouldn't the MAR say the correct number of tabs and doesn't this promote med errors? Sometime the med orders do change and the old pharm card could be wrong as well. Looking for thoughts on this. I'm orienting at a new facility and made the mistake of popping only one tablet instead of two, yesterday I took the correct dosage of this medication but did the same thing on another order that was 600 mg and the tabs were only 200 each. How can I make sure I stop making this mistake? I'm sure once I know the residents and the meds I will not make it with them but I am afraid I will keep doing it. I know I'm supposed to check the dosage ect, but just hoping someone has any good advice. Perhaps I'm not cut out to be a nurse.

I'm a new grad and I recently started working in a nursing home. A resident has A-fib and needs loprressor and has parameters to hold if under 55. I had trouble counting her apical pulse because I do not hear a lub dub in the sense that all the beats sound the same and are spaced out much more than a regular lub dub. I'll hear two beats than three, than two than 4 and it's very irregular. I'm unsure if I count each beat I hear or if one sound is lub and the other is fun. Hard to explain the sounds but it's like beat...beat......beat...beat...beat....beat.....beat..beat....beat...beat... So the dots represent that the spacing is not even at all where normal hear best is lub dub..lub dub..lub dub. I never had the opportunity to have clinical on a cardiac unit and I'm just not sure how to count the beats in this situation. I'd really appreciate some help. My personal pulse ox gives me a very low pulse but I know this is incorrect and that I need to count the apical pulse for 1minute. I also noticed that if she sits up her heart rate will speed up greatly. Please help! Thanks!

Thanks for your response, funny you put you love to learn, I do too, I'm always looking things up that I don't know or am unsure of, it in my nature. I have an interview for ED next week! Hoping I can make a good impression. I also got a call from Red Cross Finally they will have me go through training and then I will be able to select times and days when I am free to help out. Feeling happy, but nervous for the interview. I always get nervous in interviews. Wish me luck, and ill let post how it goes once I find out.

Ouch! Thanks for pointing out HIPAA, I will now never write that wrong again! I feel embarrassed I've sent out my resumes with that on it. Even my nursing instructor didn't pick it up. How were you able to point out that your not too old to learn. Obviously anyone who gets into and out of nursing school has had to learn a lot. Too bad they don't look at in that way. Do i need some examples of how I'm teachable? Just curious how you broach the subject. Thanks for all the tips. My resume keeps evolving. I've now been accepted for my BSN school so I'll be adding that. I've also signed up to volunteer with the Ted cross they've been very slow in getting me something to do.

Awesome responses, thank you for your thoughts. If I have applied to a BSN program am taking transferable courses now but have not yet been accepted do you think I can put that down as well or do I need to wait until accepted? The school is waiting for my high school transcripts which are taking forever. I've also applied to volunteer for Red Cross and have gone through the whole process just waiting to get an assignment so no way to get that on there until they get me something to do.

Thanks for your input. So, should I have a two page resume? Or some kind of different format that doesn't list all skills in each position, not to list same old, same old skills. Mostly customer Service. Won't it make me look too old starting with 1986 summer hire job. Or 1987-91 cashier and shift supervisor. I also left off teller on my resume when I first started banking in 94. Thanks again.

Advice needed for New grad with neuroscience outpatient interview
Hi, I recently graduated and have an interview for an outpatient neuroscience clinic next week. I know someone who works there and they are putting in a good word for me. I really want this position and I want to interview well. I know more experienced nurses are interviewing as well. I've only worked for one company before for many years and moved within the company, but I haven't had to do a very formal type interview. This position will be a panel of 3-5 providers that work there and all interviews will be held on the same day. From what I understand this position is mostly patient teaching, handing phone calls and helping with paperwork. As well as delegating to the MA's. There are 2 MA's and at least 5 providers maybe more, some of which are NP's. I'm sure I'll have to help the MA's at times especially if one is sick or on vacation. The MAs take care of setting the patients up in the rooms, taking vitals etc. I don't think there is another nurse in the office. I have no past medical experience, but from day one I always wanted to work in an office or clinic. I was going to tough it out for a year because I thought I would do better if I had some hospital experience first outside of my clinical experience before working in an office but I really don't care for working in a hospital setting such as med surge, I really don't like it. I have worked in office settings performing both clerical and customer service roles for many years before I became a nursing student. Here are some concerns. Being the only one responsible for the calls (I hear there are a lot of calls) While I've had to field calls at my previous employer, I know these will be different in nature. What can I expect? Any advice on how to be prepared should I get the job offer. Will I need to do a lot more reading or take a special class for this? I'm a little nervous with possibly not having someone to ask if I am unsure of what action to take when i triage a call. Besides this I could just use some advice of how to interview well with a panel and make a good impression, also for this type of environment what type of questions might I be asked? Any tips that anyone has to help me secure that position. Also any warnings about maybe not going straight into an office setting as a new grad? One last question what are some good questions to ask in this kind of environment. I don't want to go away without asking questions. I'm not even concerned about pay, I know the starting pay is an increase from what I made before and I would never ask about pay or hours. I am not sure what would be good questions to bring up.
