  1. It is a very demanding job. I am the only Care Coordinator in a practice of 16 providers and 6 NP's with 7000 Medicare patients that have Chronic Diseases. I have set up their Transitional Care Program and its running fairly smooth with the process I have in place. Now my next bog task is to establish the CCM program which is slightly more complicated. On top of that I also try to keep trying of their Home Health patient and work with billing to bill for those Care Plan Oversights. I love the job, just sometimes I get slightly overwhelmed and I am alone at this point. They are looking to get me 2 people under me to help. In the mean time I need to set up the work flow for Chronic Care Management and I would love to hear how other practices do it.

  4. Yea, I am in the same position. Its a great opportunity for growth, however lots of brainstorming. I've got the TCM figured out with not having any Care Coordination software yet. It took me about a week to figure out where the issues were within the practice. I took it from there and built upon it. Some providers are not very happy with the changes because they have to document according to certain guidelines. How has your road been so far? What challenges are you facing?

The practice that hired me is new to this and want to have a Care Coordinator on board.They do not have any software that would help me with that just yet.I am doing mostly telephonic,also transitions in care.No Home visits or clinic.I am trying to get started but all i am working with right now is AllScripts.The practice is open to purchase a software.which one do you use if any?
  6. Hello, I just started to work as a Care Coordinator for a large private practice in Virginia. I would love to network with other Care Coordinators and learn different ways of dealing with Medicare/Medicaid Care coordination of patients with multiple chronic diseases.
