Home visiting is an integral component of many communities nursing roles, particularly domiciliary nursing, hospital in the home nursing, aged care assessment teams and hospital outreach/follow up programs. During a home visit, issues can become clear that do not surface when a client attends a hospital or health center. Home visits are sometimes the only way that appropriate health services can be delivered, particularly to disadvantaged populations and individuals. Care or services provided at home may be more appropriate for specific types of clients. People maybe to sick or too depilated to leave their home, or they may not have personal, structural, or financial resources to access health services. Home visits may allow a more through assessment and individualized planning for service delivery and care. This has been found to be effective in improving client outcomes and cooperation. Visiting a client at home brings into play a whole new set of skills and issues. An important aspect of home visiting is managing time and resources to achieve the best outcomes. Prior to attending a home visit, it is important to plan. Arriving for a home visits without having read case notes properly, access information about client’s condition, or without a vital piece of equipment will waste time and resources. To prepare for a home visit for Mrs. Azizi, the nurse should plan carefully. The following should be done to prepare for Mrs. Azizi home visit which include: reviewing Mrs. Azizi referral forms, case notes /assessment forms. Addressing the reason for the visit (providing postoperative care, particularly dressing changes) and identify a goal for Mrs. Azizi during the visit. Gathering appropriate equipment required for the visit (dressing supplies are required to be organized for Mrs. Azizi visit). Consulting with team members, particularly where client/family have complex problems or are receiving services from more then one health profession. Establishing contact with Mrs. Azizi or her family prior to the visit. Planning an appropriate schedule and routine for Mrs. Azizi and reviewing safety issue and notify the agency of the visit and expected schedules. Also the community nurse needs to be aware of cultural differences and being respectful to the client’s beliefs and values. Also making aware of Mr. Azizi’s lack of English speaking, it might be appropriate to bring a translator or asking one of the family members who is can speak fluent English to assist the community nurse when communicating with Mrs. Azizi during the home visit. At the completion of the visit, the major issues addressed can be summarized with Mrs. Azizi and her family, Mrs. Azizi achievements can be identified and planning for future visits can also be scheduled. Other points that might be considered at the end of Mrs. Azizi visit that the community nurse should do include: creating an SMART action plan and creating a goal for Mrs. Azizi from gathered information from the home visit. Educating Mrs. Azizi and her family about wound management and post operative care. Referring Mrs. Azizi to specialists for further review and inspection of her wound and a follow up with her cancer treatment. Also referring Mrs. Azizi’s to other health services and community services that maybe relevant to her situation such as support groups. Evaluating Mrs. Azizi current condition and reporting back to other health team professionals that might be involved in Mrs. Azizi’s care. Completing a full risk assessment identifying any hazards that may cause harm to Mrs. Azizi or poses as a risk. Lastly finalizing any documentation including: written reports, and finalizing case notes for Mrs. Azizi for future reference. does this sound ok for this question

  3. could be since her house is on stilts and has stairs might pose as a risk, also living on a large farm isolation if something happens, also toilet being 15 meters away, also just had surgery on for the removal of her bowel cancer, and is at risk of post op complications. she is independent and is cleaning and moving furniture around while she is ment to be recovering. also isolation and being remote rural, area 45 mins away from in-town services and from medical services can be otheer risks for mrs azizi

  7. i have found a good community nursing book :) i have discussed what primary nursing is and have talked about primary principles in relation to mrs azizi. such as access to resources due to living in a rural community an not begin able to drive etc. cultural competency du to mrs azizi being newly to Australia and has limited english and also using traditional medicines question two i talked about safety and risk issues for nurses within the community such as incorporating clinical governance, future risks and hazards having trouble with questions 3-4

She lives with her husband and 2 adult children, their partners and grandchildren. They live on the large family farm, a 45 minute drive from town; Mrs Azizi does not drive. The farm house is very old and built on stilts, with 6 steps to the door. The toilet is outside, about 15 metres from the house. Mrs Azizi has been in Australia about 5 years; she has limited English, understanding more than she speaks. She uses many traditional medicines to manage her health. This is the first time the community nursing agency will be seeing Mrs Azizi. You have reviewed the hospital discharge assessment information which has identified Mrs. Azzi as being an at risk client. She has had surgery to remove bowel cancer and you are visiting her to provide post-operative care, particularly dressing changes. Wound dressing supplies are required to be organised at the expense of the client. The community nurse's visits are covered by the discharging hospital. When you pull up to the house the outside of the house is neat and the garden is well maintained. You cannot see another person in the surrounding farm land or in the surrounding paddocks, although you notice many shoes at the front door. You assume other family members are working outside on the farm. When entering the house you notice the lounge room is clean and tidy with nothing on the floor. Mrs. Aziz is vacuuming the house and moving furniture; she is not wearing shoes. There is one couch to sit on. You are offered a drink and food. During the visit three males return to the house. When you are leaving a man puts $50 in your hand. in need help answering these questions related to case scenario. and need help finding good articles and books related to community nursing , its in an essay format and if any one can help1.
