If not you have to request certified true copy of your tor then attach the cgfns form then send it together with your attestation and signed authorized prc as well.

I sent my documents thru post office. And my cgfns received it. Just allow approx. 1-2 weeks then check it online.

Im so sorry but Im still confused why do they ask the applicant to notarize the document and not send it to them?

Hi everyone, im thina, a registered nurse here in Philippines and i just started to apply for cgfns cvs for ny nclex. I am actually confused on the process. I applied thru cgfns connect, printed the documents, paid via credit card. Now, my school sent my transcript already. I had my application notarized. I sent via mail the payment voucher at the mailing address found at the header part of the document. Now, i dont know what will i do with my application form coz i tried to send via LBC my application for but they told me they cannot send it coz of the the mailing address it is in a PO box. Will i send it to tptheir PO box mailing address? Where can i send my document? Also regarding my license verification do i need to go to PRC for them to send my authetication of my license? Please help me. Thank you.
