  1. Things to consider when doing NANDA's for baby... What was the baby's gestational age? if early..there are some easy NANDA's if baby was fullterm then where there any complications with the delivery (shoulder dystocia, problems with the umbilical cord, problems with the placenta, positioning of the baby--breach, etc.--, second stage of labor lasting longer than 2 hours, etc.) knowing about the delivery can help form NANDA's for baby, and mom for that matter. Another thing to look at is mom's nutrition/health during pregnancy--did she have gestational DM, Preeclampsia, eclampsia, did she have prenatal care, ect. I hope this helps :-) Good Luck.
  2. I hope its not too late but here are some possibilities. Since I don't know anything about you patient here are some general NANDA's for the disease process. The trick is too keep it simple. 1) Acute confusion r/t dehydration and electrolyte imbalance...(finish it off with the aeb---signs & symptoms, labs, and other findings) 2) Risk for electrolyte imbalance r/t effect of metabolic state on kidney function....
