  1. In my 5 years as a Psych RN, most of my cutters/swallowers have been frequent fliers on 1:1 observation because of their danger to self behavior-swallowing non food objects and cutting themselves. Most of these patients have Borderline Personality Disorder, love to manipulate and be disruptive. I have had as many as three of these patients on the unit at once and they are exausting to care for. The last one body slammed a door and broke it in half.
  2. Patients that ask to be strapped down or make excessive demands are trying to control the staff and remain in control. These patients feel impowered when staff does what they want them to do. Know when you are being manipulated.
